About the Donated Dental Services Program:

The Donated Dental Services (DDS) program helps people who need dental care but cannot afford it by connecting them with volunteer dentists and labs across the United States. Comprehensive dental treatment is provided at no cost (we do not provide cosmetic dental care).

Application process

Please fill out the entire application to the best of your ability. Do not leave any sections blank and submit DDS application when you're finished.

You will be placed on the waitlist, in the order your application was received, if you appear to meet general eligibility requirements (this does not guarantee that you will be approved for services). You will receive notification that your application was placed on the waitlist within a month of submitting.

If you are not eligible, we will send you a denial notification with alternative resources that may be able to assist.

Depending on where you live, the waitlist can be several months to a year or more. State DDS Coordinators are unable to give wait list estimates and cannot return phone calls about where you are on the wait list.

When your application comes to the top of the wait list, a state DDS Coordinator will contact you to view your application. If you are eligible, you will be referred to a volunteer dentist for an initial consultation.

Final acceptance into the program will be made after the first consultation appointment with the dentist.

Eligibility Guidelines

You must meet ONE of the following criteria:

  1. 65 years of age or older
  2. Permanently disabled
  3. Require medically necessary dental care

In addition to meeting one of the above criteria, further eligibility requirements are below:

  1. You can only receive services through the donated program once
  2. You must exhaust all available dental insurance and benefits prior to receiving care through the donated program
  3. Treatment is at the discretion of the volunteer dentist: services like sedation, implants and complex care may not be provided.

Financial Need:

  1. You must not have the financial means to afford dental care
  2. You must exhaust all available dental insurance and benefits prior to receiving care through the donated program (including Medicaid benefits).

What You’ll Need to Know to Complete the Application:

  1. Contact information for another person in case we cannot reach you.
  2. Physician name and phone Number
  3. Medical and disability information (general diagnoses and conditions)
  4. Current dental issues and concerns
  5. Income and expenses (itemized)

Note: You can save your form progress at any time.

Supporting Documentation:

You may need to provide further information after submitting the application (if applicable).

  1. Breakdown of benefits for dental insurance or letters of denials from insurance for needed services
  2. Any treatment plans (excluding implants) from dentists that have been run through insurance to show what you would owe for care outside of the insurance coverage.
  3. Social Security Disability award letter (if you are under 65 years of age)
  4. DD214 (for veterans)
